NDIS Services

It is important to note that this program sits outside ALL OTHER Life Stream Australia programs, and at no time is it appropriate for the NDIS team to encourage a family member or individual to make decisions that positively endorse Life Stream Australia.  The services and supports an individual or their family chooses to access may or may not include Life Stream Australia services, or a choice may be made to access some, but not others.

​Unique opportunities and to “think outside the box” to help an individual’s funding package stretch further than ever before.

Connect families choosing to self-manage, so they may provide each other with formal and informal supports, mentoring and guidance.

Offers families the flexibility to seek additional supports when other aspects of their lives become a little overwhelming.  This flexibility is as easy to organise as sending an email or making a phone call to the NDIS team.

​Our NDIS team operates within the core values and beliefs of Life Stream Australia.  That is, support is offered and provided wherever possible to reduce the impact of the additional responsibilities of not only caring for a family member with a disability, but the responsibility of managing the decisions, coming up with the ideas, and “thinking outside the box”.

NDIS

Life Stream Australia is a registered service provider with NDIS, with approval to deliver a wide variety of services, including, but not limited to:

• ​Direct service delivery
• Coordination of supports (Supports Connection, Supports Coordination, Specialist Supports Coordination)

For all NDIS Participants, whether self-managed, plan managed, or NDIS plan managed, the NDIS services are responsible for the preparation of service documents, agreements, support documentation and making claims for payment.

NDIS services remain informed and up to date with the frequent changes at NDIS, and can provide Participants and their families with information, resources and support as they transition through the process from traditional models of funding to NDIS, and as they engage in their responsibilities to control and direct the use of their NDIS funding package.

The NDIS services offer Participants, families, and other members of the community access to

• Information and consultation
• 1:1 planning, assessment and transition preparation
• Small group practical based workshops, as information and needs change
• Information sharing through

• Access to experienced, skilled and supportive staff

Life Stream Australia and the NDIS services are proud to support Queensland communities through this process and their ongoing access to NDIS.
 
Life Stream Australia’s NDIS services provide support, guidance, information and financial accountability for Participants of the National Disability Insurance Scheme (NDIS).
