NDIS

The National Disability Insurance Scheme (NDIS) is an Australia-wide project that will support hundreds of thousands of people with significant and permanent disability, as well as their families and carers.

The National Disability Insurance Scheme significantly changes the way disability services are funded and delivered across Australia. This nationwide reform puts you at the centre of the decision making process about how you want to live your life, your choice of supports and who you would like to deliver your supports. It is designed to provide you with choice, control and independence.

It should bring you to a short, 5 question form that will ask you about your concern. Nothing is too small, if it is worrisome for you, it is important to us. We can only improve our services if you tell us what isn't working.

Life Stream Australia's Incident Management Process

We are committed to ensuring the safety of all Life Stream Australia participants and staff and have a thorough Incident Management and Abuse and Protection Response in place to ensure all staff are aware of what to do in the event of an incident. This response covers who needs to be notified and how this is managed and followed up afterwards. We consider all incidents and reports of abuse or neglect to be treated seriously and action in accordance with our obligations under the NDIS Quality & Safeguards Commission. A copy of our Incident Management Policy, Concerns Policy or Abuse & Protection Response Flowchart can be made available at your request. Please contact your local Service Coordinator.

NDIS Services and Supports

Life Stream Australia charges as per the NDIS Pricing Arrangements and Price Limits Guide, as released by the NDIA each year.
Each Participant will be provided with a detailed, individualised pricing of the support they have requested (Schedule of Supports), noting the total cost of service delivery.
